"Notification of Change" (Form 9)
If you fail to notify us of your new address, you may not be able to receive a notice of confirmation regarding your pension benefits eligibility from our Fund and in some cases you may not be able to receive your pension! Please make sure to contact us.
Our Fund will send you the pension procedure documents when your 60th birthday coming up. Following your birthday, please fill in the required items on the documents and return the form to our Fund along with the documents below. We will send you the notification of annuity payment within 1-2 months of receiving the documents.
"Claim for Annuity" (for those who resign after July 1, 2007)
1.The fund will send the "Claim for Annuity" form to everyone.
2.Please obtain a certificate of residence or family register (original) at your municipal office. (It must be issued after your 60th birthday.) If you are not a Japanese national and non-resident in Japan, please attach a copy of your passport. Note: Mitsubishi UFJ Trust and Banking Corporation will be the agent for pension transfer.
A beneficiary who opted to receive some or all benefits in the form of a pension at the time of resignation may cancel his or her pension in part or in whole before reaching the age of 60. If you want to cancel your pension, please contact our Fund first of all. After we confirm if your pension can be cancelled based on the type of pension and what percentage of pension benefits you opt to receive as a lump-sum payout, we will send you a "Claim Form for Partial / All Lump-Sum Payment for those in a Waiting Period." We will also send you a "Declaration of Retirement Income Receipt" because any lump-sum payout you receive as a result of cancelling your pension is treated as retirement income. (TEL 078-336-6922)

We will check the details of your pension benefits, and send you a "Claim Form for Partial / All Lump-Sum Payment for those in a Waiting Period" and a "Declaration of Retirement Income Receipt."

Please enter the required information in the "Request Form (Form 7)" and "Declaration of Retirement Income Receipt" (not necessary for cancellation of additional benefits), and send them back to us.

We will calculate the amount of lump-sum payout for pension cancellation, and send instruction documents for payment of the lump sum to the bank. We will also send a "Defined Benefit Pension Plan Notification of Lump Sum Payment." To the applicant requesting pension cancellation.

You will receive a "Notification of Lump-sum Payment" from Mitsubishi UFJ Trust and Banking Corporation 5 business days before the date of payment. Please confirm the date of payment and the amount of lump-sum payment on the notification. After that, a lump-sum payment will be transferred to your designated bank account on the date of payment.
